For Participants: Passport to Enjoyment

Everyone who enrolls in the program receives a packet, welcoming them to the program. Included in this packet is a brochure, titled Passport to Enjoyment. The passport is designed to open up your world to more enjoyment by outlining the variety of activities that we are able to arrange. The passport will list each category of activity and provide a guideline of how we will be able to fulfill the request. The guidelines are as follows:

*Spur of the Moment is our reimbursement program, which allows you to do an activity without pre-planning it through our office and receive reimbursement of up to $50.

Listed below are a few areas of the Passport to Enjoyment that we feel may warrant additional explanation.

Dining

What the Passport says: 2 people, maximum of $75.00

What this means: For any dining request we will cover the cost of two people up to a maximum dollar limit of $75.00. You can make a request for a meal for more than two people, but we will only cover the cost for two.

What it does not mean: That you are guaranteed $75.00 for every dining request.

How it works: When we receive a request for dining we make reservations (if possible) for the number of people listed on the request form. The next step is to determine what dollar amount is appropriate for dinner for a party of two at that particular restaurant. In some cases that amount is more than $75.00, so, we will authorize payment of $75.00. In other cases, that amount is less than $75.00 and we will authorize payment of less than $75.00. For example:

"Sarah" requests brunch for 4 people at "Nature's Garden." The cost of brunch is $16.00 per person. For this request we would make reservations for a party of four, but make arrangements to pay for two of the four brunches. In this situation, the amount authorized would be less than $75.00.

Therefore, please feel free to make requests for meals with groups larger than two or, for more expensive restaurants, with the understanding that we will be able to cover as outlined above.

What the Passport says: 2 people, maximum of $100.00

What this means: For any ticket request we will cover the cost of two tickets up to a maximum dollar limit of $100.00. You can make a request for more than two tickets, but we will cover the cost for two.

What it does not mean: That we are able to arrange for four $25.00 tickets because the total cost is $100.00.

How it works: When a request is made for two tickets, we make arrangements for the tickets and contact you with the details. When a request is made for more than two tickets, you will have the option of a "pre-approval." A pre-approval allows you to make arrangements for more than two tickets for a request. In this case, a member of the program staff will contact you with the exact details about how your pre-approval will work. In every case, you will be responsible for purchasing the tickets in advance and submitting the receipt(s) to the Foundation for reimbursement. Please be aware however, that individual details will vary.

Pampering

What the Passport says: 1 person, maximum of $75.00

What this means: For any pampering request we will cover the cost of your service(s) up to a maximum dollar limit of $75.00. You can make a request for a spa day and we will cover $75.00 of the total cost. You can make a request for pampering services for more than yourself, but we will cover the cost of your service(s) only.

What it does not mean:That you are guaranteed $75.00 for every pampering request. The amount assigned will vary depending on the service.

How it works: Most pampering requests are for one service and are arranged with the spa or massage therapist requested. Requests for multiple services are scheduled and payment is arranged up to $75.00. You are responsible for paying the amount above $75.00.

Your Unique Idea

The majority of activity requests that we receive are listed in the Passport to Enjoyment. However, they are not the only categories of activities that we can make arrangements for. The program staff encourages all participants to be as creative as possible when thinking about their activities. Unusual requests we have received range from the adventurous (skydiving) to the creative (stained glass supplies) to whimsical (trapeze lessons). Please consider this an invitation to be creative! If you have a unique idea for an activity, please contact the program staff with details about your request.
